4 easy steps. Done in PSP-X

1) Open your image in psp. I chose this picture of Gemma Ward.

2) Duplicate your background layer.

3) [this step may be different if you have psp 9 and below] Select your first layer, and go to adjust->add/remove noise->edge preserving smooth.

Make your smoothing amount between 3-5.

4) Double click your second layer, and set the blend mode to soft light.

Add text/more effects if you like. I added text, and the outcome looks like this.

The skin has a lovely tone & the scratches are gone!

On Paint Shop Pro Photo XI, there's an even easier way:
Just click Adjust->Digital Camera Noise Removal...->Set the blend modes to about 50, depending on how smooth you want the picture, and click ok.
It gives you a smoother look, and it won't mess up the hair as much.
