music, html trouble Options

i feel so stupid...i had the code for inserting music but i think i ended up copying part of it and mixing it around. i have my layout html code and HUGS html codes besides my music. this is the html code i have and if anybody knows the standard code i dont care if i have to start over...at lease i'll know what the hell im doing. i can fill in the blanks with all the names of hte songs and the urls for the music i just need the standard code for music.
