PS2 question!, AC Adaptor Recall

Hi, up until recently I found out that my AC adaptor for my slim ps2 was in fact one of millions that were recalled by Sony for being "defective", and also my AC Adaptor has completely stopped working, thus giving no power to my ps2. I went on the playstation website looking for a free replacement, and the form for the replacement asks me for my PS2 model # and my barcode, unfortunately this information wouldn't be a problem if the sticker under my ps2 hadn't mysteriously dissapeared, my ps2 is from 04, can you blame me? Anyway, has anyone gone through this process with the replacement of the defective Adaptors? I was wondering if I couldn't possibly find my model# and barcode(wishful thinking) another way, without the sticker? Do I really need this information? I have all the information for my defective adaptor that is required by the site. Should I just email them or call them explaining my missing sticker crisis, would they still send me a replacement? Sorry if I cannot be any clearer, it's hard to try and explain a situation like this over the internet. Hopefully someone knows what I'm rambling about. Thanks a bunch.
