Scroll, k? Options

I'm trying to add a survey I took into a scroll box code. This is the code I'm using:

I've noticed that the survey contained an excessive amount of code, so I've deleted them all. Not the necessary ones, just the useless ones that might be clogging up the scroll box code, like credits and stuff. Still, it contains a lot of script, like the <td> and <tr>.

After putting all these scripts together and saving it, I viewed my profile and saw a big empty space in the profile section I applied the script to, and the survey right below it, with no scroll box around it whatsoever. The scroll box setting is Width 150, Height 400 - I'm sure that this does not exceed the size of the survey. I'm not sure what the problem is, the scroll box code or the survey's code. Can you find it?
