Friend Request!, why does it ask for my last name or email.. Options

i need some help!

i created my own layout and i dont know if i did something wrong, but when ever someone tries to add me as afriend it ask them for my last name or email.

when in my privacy settings i did not pick this setting. also, when someone does type my correct last name or email it shows as incorrect!

what do i do please help!

Remove the space located before your friendID in each URL.
