Creating a personal contact table, tell me where or how i can create one! Options

I was wondering if anyone knew of a site or place to creat a personal contact table. I've seen a few on people's pages but never where to create one. One girl had a picture in the background with the add me write me stuff on top. I loved it. I want to make one! helP?
