Problems with my video code on Band Div Myspace, Just getting a white box Options

Hi Guys, Gonna post my code etc. - basically I'm only getting a white box at the bottom of my profile. Position is no problem, I just need to be able to see the video. Cannot host it on YouTube as they reject it for being too long. Please help as I have to get this launched by this afternoon!! Best wishes, T xxx
