Comment Box, the script on here... Options

I tried using this code:

to make my own.
(I don't know how to add links)

But after I put it in my about me.
&changed the things to how I wanted, it wouldnt show up.
Also it didnt have the edit&submit buttoms like normal to either go back or submit it to be final.

This the code I entered into my about me:

I only wanted the submit button, that's why I had the reset button all white...

But is there something wrong in this to make it not show up?
