i would say 'forgetting an important event' .. but to be realistic.. you'd have to be really absent minded to forget a birthday or big annivarsary...

so i will say that a common mistake is not voicing your concerns... like keeping things that bother you bottled up til one day it just bursts and you give up on the relationship

Criminy, everyone's said most of the ones I'd think of.
The only ..uh..uncommon ones I could think of were:
Lack of intimacy (not necessarily sexual) and not knowing the person terribly well before you started a relationship with them.

...holding the relationship with closed hands (metaphor)

meaning... holding too tight unto the realtionship as tho it means everything to u. it may seem that this is good because it means that you love ur partner more, but it will only cause to jelousness wen he.she talks to someone else, or obsession. obession= not good in a way because you will lose interest faster and if something bad happenes, you will fall hard and sometimes get hurt really badly...

also, you could possibly annoy ur partner if your too overprotective. not a good sign... keeping the relationship loose and fun in the begining will always be the best. ^_^ or at least for me

most common reasons:

jealousy
lack of trust, resulting in the jealousy to take control
miscommunication
differences in how people go about things, like arguing, one person might be loud wheree the other one gets very emotional with loudness etc then it just dont work out
too much time together
or not spending enough time together etc
oh yeah, and selfishness and not listening, lol

Breaking up over stupid reasons like mistrust or miscommunication [etc. etc. etc.]. Or maybe, just lack of commitment.
