For the words at the top, do you want a banner or just words? If it's just words, you can use this.

or you can have a background, then make a banner with a transparent background that has words on it and put it at the top I did mine like that, if you do make a banner that you want at the top then put this into custom html header. replace PIC URL with your pictures URL. If you don't know where to upload a picture to get the url, go to Imageshack
