Did you use Fantastico to install your script?

What you can do, is just delete it via FTP, or you can go into your "File Manager" and navigate to where the script is located, and uninstall it that way.

If it uses a mysql database, remember to delete the database, and database username since you don't need it anymore :)

A mysql database has nothing to do with a script.
What the above means is that you cannot create another database, and if you have only one database, that means you can only use one database.

You can use a one database for more then one script.

Just type in the database you already have created, and use that for your script.
