you should tell her that you like her and then try to find out if she likes you. and if she does, you should do suhmthin rememorable; maybe bring her roses or wuh ever her fav flower is and with the note attached, say "will you be my girlfriend" or wuh ever you want it to say askin her to be your g/f.
