Hm, well, I think it's the edges of the picture that kind of bother me. Try cleaning that up; make it a bit more crisp instead of blurred. Also, Maybe make the black paint a bit more realistic? Like blend it into the background by using Overlay or Soft Light. Also, if possible, may be try to make the paint match the blood dripping from the woman's mouth (since that blood, too, looks like paint).
