Need help setting up data base, data base and my sql Options

I need help with setting up my data base. I watched a video on a link and in the video the person did not explain how to set it up but he explaind how to configure all of my registration files. I'm trying to create a C-Panel log in.

Here is the code that I asuming needs attention, but I'm new to setting up data base.

Also I'm sendin a screen shot of were I'm at with my data base hoping this may help you to help me.

Can someone please guide me in the right direction?
