quick help w/ comment box! Options

Alright here is my code for my comment box...the problem is i wanna take out where it shows up on my myspace create your own comment box i dont want that showing up. so what do i delet in the code so this doesn't show up? also i only want a submit button and not a reset button so what else do i delet..i tried doing it but i ended up just messing the code around..thnx!
