Comment Box Not Working Options

My little comment box is giving people errors. Can anyone figure out why? Ive about had it..

I didnt put that there, no. Uhm.. but every so often they wont be disabled and people can leave me comments. They have to use the ADD COMMENT link though because the box wont work. Ive checked and other peoples boxes work.. even If i use their code with my FriendID.. its not workin for me for some reason.

Could it possibly be something outside this code? I couldnt imagine what but I thought Id ask.

And I might add that I've tried different variations of the form code, like I said, even from friends perfectly-fine working boxes. Its just strange.

Try it. It does something TOTALLY different than it would if it were because of the disabled comments.
