contact table, how do i do this?? Options

Im just wondering how do you make your own cantact table? But not the kind you have to make.. like the kind that have already been made for you that have pictures and stuff on them. thanks!

ok, I'm using this code for a new comment box that I got off that website:

For some reason it is not working for me. Am I doing something wrong? I put it everywhere in my about me section also. This is the only thing I have been having trouble with, and I've been working with it for days.
