I have been friends with a guy since preschool. We pretty much grew up together. But you see I have this thing about people who smoke, where I really dislike them, and he does. And he's 13. So should I tell him to kick his habit, or it's over? or should I deal with it, even though it will really hurt my feelings that he's hurting himself?

I don't think you should just stop being his friend because he smokes, seeing as how you've been friends for a long time....just slowly and calmly help him stop the habit by getting him help and telling him the side effects and consequences of smoking. Show him the statistics of what happens to people who smoke and how their life span is cut in half....and how they eventually will have cancer and cause harm to the people they love.
