Affiliates button, please help Options

Hi, i was wondering if anybody could tell me how to make an affiliate button? I know how to design it but what is the code I need for others to be able to take and put on there site?
Example:

Also, if someone could give me the code where you put a code inside a box with a scrollbar. Sorry, im really bad at descriptions. But if anyone can help, thanks so much!

Put the codes in your website stats for it to be on the bottom.
