i don't understand how there can be that many or it can go on for that long without someone turning him in...

He was a pediatrician. Maybe they were too scared to tell? But still someone would have eventually told someone.

They do usually, at least they ask "do you want your mom/dad to come with you?" and i would assume that a little kid would always want their parent with them...at least i always did.
