Yes, I would like to make make one exactly like that and add it onto my wordpress contact page Here.

I was messing around with some javascript code and this is what I got HERE, but the form is not functional. Also I found this SITE but I don't know how to add the forms into my wordpress pages?

Thank You for your help. I have found that cforms is the best way to go and help's with the rounded corners with a little bit of editing. *smiles*
