I installed Affiliationally correctly, then when I added my category.
This is what I get

go to that file and open it in an editor that has numbered lines and go to line 1051. then post the code on that line.

also line 19. i missed it.
and you may be pointing it wrong. you dont need a slash after index. php.

also if you could post some documentation that would help
