It was most likely done with a camera using a slow shutter speed.

You could take one picture and keep it at full opacity, then put the screamy picture next to it at a low opacity. Add some motion blur and there you go.

^ true, you can also use that technique with several images to get a more real look or play around with it.
The smudge and blur tool can also help with the curve.
