There are several cursor-producing programs out there, but you have to pay for most of them. There are, however, free trials that last for a period of time. ArtCursors and IconForge are free-trials I've used in the past.

As for stroking text on Photoshop, you achieve it by following these steps:
1. Make sure the text layer you want to be stroked is selected.
2. At the bottom of the Layers panel, you should see a small button with a lowercase "f" inside a little circle. Click it and it opens a dropdown menu.
3. Select "Stroke." From there on, you can edit the settings of the stroke the way you like it, and then you can apply it.

to stroke, click BLENDING OPTIONS on the layers pallette, then click STROKE>

can't you make your text a brush and set it as stroke?

You can just select the text, create a new layer and then right click and pick stroke from the drop down menu. The marquee tool must be selected when doing this though! This way you can stroke more than once too.
